Integrative Primary Care – is healing-oriented medicine that takes account of the whole person (body, mind, and spirit), including all aspects of lifestyle. It emphasizes the therapeutic relationship and makes use of all appropriate therapies, both conventional and alternative
PCMH – Patient Centered Medical Home. Care is coordinated with a team of health care providers care – from doctors and nurses to wellness coaches, mental health specialists, and beyond. The team works together to anticipate the patient’s needs, communicate their findings with each other, and make sure no aspect of the patient’s health slips through the cracks. This model improves the quality of health care and decreases costs.
Team-Based Care – This approach is highly collaborative and requires excellent communication skills on the part of all the health care team members. A patient at CHI Health Care has the benefit of a team of providers working together to optimize their care.
