We prioritized taking commercial insurance, Medicare, Medicaid, and using a sliding fee scale. A key part of our business plan relied on insurance payment reforms known now as “value based care.” Healthcare has traditionally used a “fee for service” payment structure that pays for treatment only after you get sick. Value based payments compensate providers to get people healthy and keep them healthy. And this is what CHI became successful at, fostering health. We knew we were doing something right as over the years we heard, time and again, from our patients, “This is how health care is meant to be.”

Unfortunately, the payment reform movement has hit many obstacles along the way and value based programs have not taken hold fast enough to sustain CHI’s financial survival. Despite making drastic changes and exploring many partnerships, it is with great sadness that our Board of Directors has decided to close the clinical practice as of July 23. Although not in clinical practice, CHI will endeavor to move health care toward its vision.

Please note: as of August 12, CHI Health Care can no longer accept medical records requests. Patient medical records may be requested by the patient through Iron Mountain. Below is the contact information to send the medical records request forms. Please reach out directly to Iron Mountain for assistance.

For our patients:

Below is a list of potential primary care providers in the vicinity. Please be aware that as far as we know, there are no other practices in the area that accept insurance AND offer the full scope of integrated primary care you have become accustomed to at CHI.  As of early May, these practices all indicated they were accepting new patients, but be aware they may not accept pediatric patients (kids!). Be sure to ask when you call. Inclusion on this list is not an endorsement of the practice or a guarantee that they accept your insurance. It is YOUR RESPONSIBILITY to verify insurance coverage, both with the new provider and your insurance company, and to notify your insurance company of your new primary care provider if needed.
